2020 Anti-Terrorism Compliance and Charitable Status

Heightened national security concerns in the United States require that United Way of Kenosha County (UWKC) institute a comprehensive compliance program designed for the detection and prevention of the flow of funds to terrorists and/or terrorist organizations.  This compliance is in agreement with the USA Patriot Act.  We ask that you cooperate with this compliance program by completing all of the required information on this 2020 ANTI-TERRORISM COMPLIANCE AND CHARITABLE STATUS form and clicking the 'Submit' button.

Your organization’s response is required in order to receive funds and/or designations from our 2019/20 United Way Campaign.  We need to collect this information each calendar year from all agencies receiving designated donations or COVID-19 Response grant funds.  We ask that an authorized manager complete this form.  This information must be completed and submitted or you will not be eligible to receive designated donations or COVID-19 Response grant funds through UWKC.
